Cerebral HIPAA Marketing Authorization

By selecting “I accept”, I authorize Cerebral to contact me via SMS or email for advertising and marketing purposes and to share my email; certain persistent identifiers (such as IP address(es) or device IDs); City and Zip Code associated with my account; how I interact on the website; and my long term value to Cerebral with third-party websites (such as Google) and social media platforms (such as Meta & TikTok) to improve Cerebral's platform and services.

I understand I can revoke this authorization at any time in writing, except as to prior uses, by emailing support@cerebral.com. I understand that disclosed information may be re-disclosed and no longer protected by HIPAA. I understand that this authorization will expire two (2) years from signing this Authorization or two (2) years from the date of last service received, whichever is longer. Signing this Authorization is not required for treatment, payment, or enrollment. I will receive a copy of this authorization via email, equally valid as the original, after accepting it.
