The purpose of setting goals is to positively shape your thoughts 
and actions by providing motivation toward achieving your desired outcomes. Meaningful goals not only offer a sense of purpose but 
also guide you in various aspects of life, including social, personal, career, and family.
This worksheet is designed to assist you in exploring both short-term and long-term goals, while also offering a way to track your progress. By monitoring your progress, you have the opportunity to reflect on where you are currently, where you want to be, and what you might need to get there. 
Although we encourage you to share and work on these goals with 
your therapist, you’re also welcome to complete this worksheet independently and track progress on your own.
This worksheet is not meant to be used as a clinical diagnostic tool and you should meet with a qualified mental health professional for a treatment plan unique to you.
